Clinical Study of Obstructive Jaundice


Abstract
Obstructive jaundice that is jaundice caused by any mechanical obstruction, compared wit one caused by hepatocellular disease or bold dysclasia.
So, we can say early diagnosis and operation make patient survive. and adjust operation make them comfortable. 57 cases of obstructive jaundice who received surgical intervention were evaluated from March 1985 to February 1988. in Seungbuk Jungang Hospital.
1. Total number of surgical inpatient for hepatobiliary disease were 180 during same period, and number of patients who had operation for obstructive jaundice were 57(31.7%)
2. 44 cases (77.2%) were benign, 13 cases (22.8%) were malignant and 46 cases (80.1%) were accompanied by biliary stone.
3. Age range was 24-83 years old(average 54), average in age was 59 in malignant ant 51 benign cases.
4. The sex ratio of male to female was no difference in total cases but 1:1.4 in benign an. 5.5:1 in malignant disease.
5. The commonest clinical symptom was RUQ pain(50.5%), jaundice(20.3%) in second an. fever (10.3%) in third.
6. In almost all the cases, direct biliruvin was markedly increased and the average of to -
biliruvin was 7.63mg%, 13.3mg% in malignant and 5.95mg% in benign cases.
7. Alkaline phosphatase, GOT and GPT were increased more in malignant diseases but has
no significant differences.
8. A / G ratio was reverced in 17 cases (20.9% ), 16 cases (94.1%) were reverced in benig
cases and were accompanied with common bile duct stones in all.
9. E-coli was the most common microorganism in 17 cases(35.4%) and 12 cases(25.Q%)
were negative in bile culture.
10. Clinically cholangitis symptomes were appeared in 17 cases(20.9%) that had common bil:
duct stones in all. 7 severe cases(12.3%) were benign diseases and acute suppurativ
cholangitis with frank pus in common bile duct were 3 cases(5.3%).
11. The most common coplication was wound infection in 7 cases(25.0%), and pulmonary complication was second in 5 cases(17.9%).
12. Average common bile duct diameter was 1.96cm and had no difference in malignant and benign cases.
